- CONCLUSIONS: This case demonstrates how subacute, normotensive cardiac tamponade can present subtly and be easily overlooked. In patients with persistent tachycardia and unexplained effusions, particularly when initial imaging and pleural fluid cytology are inconclusive, early echocardiographic assessment and pericardial fluid analysis are essential. Multidisciplinary collaboration enabled timely identification of an underlying malignancy, reinforcing the need for a high index of suspicion when evaluating hemorrhagic pericardial effusions.

- CONCLUSIONS: Current evidence suggests that rifampicin has biologically plausible antivirulence activity against HvKp/HmKp, mainly through suppression of capsule-associated hypermucoviscosity, and may have potential as an adjunct in selected severe or persistent infections. However, the available clinical evidence is sparse, uncontrolled, and heterogeneous. Rifampicin should therefore be considered hypothesis-generating rather than established therapy. Further mechanistic, pharmacokinetic, and prospective clinical studies are required to define its role, optimal combinations, dosing, safety, and clinical effectiveness in HvKp/HmKp infections.

- CONCLUSIONS: Frail elderly CAP patients exhibit immune imbalance characterized by elevated Treg proportions and Treg/Th17 ratios, which may be associated with their poor prognosis and suggests potential value for immune-targeted interventions. The dysregulation of cytokines TGF-β and IL-17A aligns with the changes in Treg/Th17 proportions, potentially playing a role in the regulatory mechanisms of frailty-related immune imbalance.

- CONCLUSIONS: In this pilot study, ω-3 FA supplementation at 3.50-8.75 g/day to enterally fed patients with severe pneumonia for 7 days was relatively well-tolerated, shortened days of MV, and decreased hospital cost. Further investigation with adequate statistical power and larger sample size is warranted to confirm these clin-ical benefits and establish the optimal dosage for this supplementation strategy.

- CONCLUSIONS: M. pneumoniae isolates collected in the Republic of Korea during 2023-2024 exhibited high rates of macrolide resistance (89.4%), with ST3 as the predominant genotype. Distinct MLST distributions were observed between resistant and susceptible strains, underscoring the need for continuous molecular surveillance to monitor periodic outbreaks and shifts in genotype.
