- CONCLUSIONS: Severe Legionella pneumophila pneumonia may progress rapidly to ARDS, septic shock, and multi-organ dysfunction in elderly patients with major comorbidities. In this case, BALF tNGS provided etiological evidence after conventional microbiological testing was nondiagnostic and helped support pathogen-directed antimicrobial adjustment. Early consideration of Legionella infection and timely use of lower respiratory tract molecular testing may help shorten diagnostic uncertainty and guide antimicrobial decision-making in severe pneumonia.

- CONCLUSIONS: Prevotella oris is a rare but significant pathogen in pediatric empyema. A high index of suspicion should be maintained for anaerobic infections in children presenting with a history of dental procedures, abnormal immune parameters or possible immunological vulnerability, or poor response to empirical antibiotics. Traditional cultures are often inadequate; therefore, mNGS serves as a crucial tool for the early detection and precise treatment of difficult-to-culture anaerobes.

- CONCLUSIONS: For patients with AIDS complicated by PCP, particularly for those with concomitant barotrauma, early initiation of ECMO support may rapidly improve systemic oxygenation and lower the intensity of MV. This approach may decrease ventilator-induced lung injury (VILI), facilitate lung rest and lung protection, and provide a therapeutic window for treating the underlying disease. Clinicians should comprehensively evaluate disease reversibility, immune recovery potential, and the severity of lung injury. - CONCLUSIONS: This case highlights the critical role of rapid molecular diagnostics in the early detection of coinfections in necrotizing pneumonia. The timely identification of influenza and MRSA facilitated targeted antimicrobial therapy, which was instrumental in preventing further complications and improving the patient's prognosis. As PCR-based diagnostics become more widely available, their integration into routine clinical practice can enhance the management of severe pneumonia cases, ultimately leading to better outcomes. Clinicians should maintain a high index of suspicion for coinfections in severe pneumonia and leverage rapid diagnostic tools to guide early and effective treatment strategies.
