- Secukinumab in Refractory Generalized Darier's Disease with a 40-Year Disease Duration: A Case Report. [Case Reports]Clin Cosmet Investig Dermatol. 2026; 19:620845.CC
- CONCLUSIONS: This case represents a severe form of generalized Darier's disease with well-defined clinical, pathological, and genetic features. Secukinumab treatment suggested partial clinical improvement, although complete clearance was not achieved. However, the patient discontinued treatment due to financial constraints, and long-term follow-up data are lacking, warranting further investigation. This report provides preliminary experience with biologic therapy for this condition, and the findings require confirmation through controlled studies.

- Opioid-Sparing Effects of Perioperative Ketamine With Inconsistent Analgesic Benefit in Orthopedic Surgery: A Systematic Review of Controlled Trials. [Review]Cureus. 2026 Jul; 18(7):e112926.C
- Effective postoperative pain management following orthopedic surgery remains a challenge, particularly in the context of increasing opioid-related morbidity. Multimodal analgesic strategies incorporate adjuvant therapies such as ketamine to reduce opioid requirements while maintaining adequate pain control. This systematic review evaluated the efficacy and safety of perioperative ketamine as an a…

- Refractory Pruritus in End-Stage Prostatic Adenocarcinoma: An Unusual Presentation of A Hypereosinophilic Syndrome. [Journal Article]Eur J Case Rep Intern Med. 2026; 13(8):007237.EJ
- CONCLUSIONS: Paraneoplastic hypereosinophilic syndrome should be considered in patients with advanced prostate adenocarcinoma and persistent unexplained eosinophilia after exclusion of allergic, infectious, autoimmune, endocrine and haematologic causes. In paraneoplastic hypereosinophilic syndrome, the severity of pruritus may not correlate with peripheral eosinophil counts, and symptom burden can persist despite corticosteroid therapy and normalisation of eosinophilia. To our knowledge, this is one of the first reported cases of paraneoplastic hypereosinophilic syndrome secondary to metastatic prostate adenocarcinoma in which refractory pruritus became the indication for palliative sedation.

- Successful treatment of Grover's disease with upadacitinib: A case report. [Case Reports]SAGE Open Med Case Rep. 2026; 14:2050313X261463981.SO
- Transient acantholytic dermatosis, also known as Grover's disease, is an acquired pruritic eruption commonly refractory to conventional therapies. We report a case of a 76-year-old male with Grover's disease presenting with persistent pruritus and skin lesions despite multiple treatments. Upadacitinib 15 mg once daily was initiated off-label and the patient reported marked improvement in pruritus…

- Pruritus Is Frequent and Persistent and Impacts Quality of Life Among Patients With Primary Sclerosing Cholangitis. [Journal Article]Aliment Pharmacol Ther. 2026 Aug 17. [Online ahead of print]AP
- CONCLUSIONS: Pruritus is common, clinically meaningful and fluctuates over time in PSC. Its strong impact on quality-of-life underscores the need for standardized assessment and development of more effective therapies.

- Polycythemia vera (PV) is a clonal myeloproliferative neoplasm primarily caused by the JAK2V617F mutation, present in 95% of patients. It results in erythropoietin-independent hematopoietic proliferation, erythrocytosis, increased hematocrit (Hct), leukocytosis, thrombocytosis, and hyperviscosity, predisposing patients to thrombotic events and neurologic deficits, including erythromelalgia and pr…

- Primary Sclerosing Cholangitis in an Elderly Female With Concomitant Gallstone Disease: A Diagnostic Challenge and Literature Review. [Case Reports]Cureus. 2026 Jul; 18(7):e112859.C
- Primary sclerosing cholangitis (PSC) is a rare cholestatic disease that causes fibrosis and strictures of the bile ducts. It is primarily diagnosed in males between 30 and 40 years of age. Its presentation in elderly females is uncommon and represents a diagnostic challenge. The objective of this study is to present a clinical case of PSC with an atypical presentation in an elderly female with as…

- A Diagnostic Challenge: Uncovering Autoimmune Hepatitis Masked by Choledocholithiasis and Unsupervised Ivermectin Use. [Case Reports]
- Autoimmune hepatitis (AIH) is a chronic immune-mediated liver disease diagnosed by the clustering of compatible biochemical, serological, and histological features after exclusion of competing causes. Concurrent biliary obstruction or drug-induced injury can obscure the underlying autoimmune process and misdirect the initial diagnosis toward a purely obstructive or toxic cause. A 65-year-old woma…

- Self-Management Ability and Its Associations with Pruritus Severity, Self-Perceived Burden, and Psychological Resilience in Adults with Atopic Dermatitis: A Cross-Sectional Study. [Journal Article]
- CONCLUSIONS: Among adults with AD, poorer overall self-management was associated with older age and greater self-perceived burden, whereas greater psychological resilience was associated with better self-management. Associations differed across self-efficacy, overall self-management behaviors, cognitive symptom management, communication with physicians, and exercise. Assessment of pruritus, perceived burden, coping resources, communication difficulties, and age-related barriers may help tailor self-management support; longitudinal and intervention studies are needed to determine whether such support improves outcomes.

- Qing-Re-Qu-Shi formula improves clinical outcomes and is accompanied by gut microbiota and microbial metabolite remodeling in adults with moderate atopic dermatitis: a randomized placebo-controlled multi-omics trial. [Journal Article]
- CONCLUSIONS: Qing-Re-Qu-Shi formula improved clinical severity and patient-reported outcomes in adults with moderate AD. These benefits were accompanied by selective gut microbial remodeling and circulating microbiota-related metabolite changes, supporting an associative link between clinical improvement and gut ecosystem remodeling rather than proving causality.

- Atopic dermatitis (AD) is a chronic, relapsing inflammatory skin disorder characterized by intense pruritus, eczematous lesions, and significant impairment of quality of life. In severe or chronic cases, AD can present with diverse atypical phenotypes, among which prurigo nodularis (PN)-like lesions are clinically challenging due to their recalcitrance to conventional therapies. - Paraneoplastic dermatomyositis-like syndrome preceding diagnosis of small cell lung cancer: a case report. [Case Reports]
- Paraneoplastic syndromes (PNS) are frequently misdiagnosed as independent neurological, endocrine, cutaneous, or rheumatic immune disorders due to their low incidence, highly variable, and complex clinical presentations. Currently, when faced with clinically unexplainable phenomena, PNS is seldom considered in the differential diagnosis. A 68-year-old non-smoking Chinese male presented with insid…
