- CONCLUSIONS: This case report highlights the importance of considering Poncet's disease in the differential diagnosis of seronegative spondyloarthritis, particularly in patients with underlying TB. A high index of suspicion and prompt initiation of anti-TB therapy can lead to rapid improvement in symptoms and prevent unnecessary investigations and potentially harmful immunosuppressive medications.

- CONCLUSIONS: The presence of established SIJ pain risk factors did not predict improved response to intra-articular steroid injection when using t-test for comparison. When using Cohen's d test however, a small effect size of (d = 0.28) was shown for increased PF following SIJ injections in patients with preconditions compared to patients without preconditions. This finding may indicate that patients with the preconditions investigated in this study can be expected to have a marginally better physical function following SIJ injection compared to their peers without preconditions. These findings suggest that preconditions such as fusion, arthroplasty, or inflammatory disease may influence injection efficacy. Larger multicenter studies are warranted to validate these results.

- CONCLUSIONS: This case highlights the importance of considering ochronosis in elderly patients presenting with joint degeneration and intervertebral disk calcification. Characteristic pigmentation observed intraoperatively should prompt further investigation. Early recognition is crucial to differentiate from other degenerative conditions and guide appropriate management. Clinicians should maintain a high index of suspicion, particularly when supported by clinical history and radiological findings.

- CONCLUSIONS: The normal spinal enthesis harbours an immunoregulatory cellular environment related to entheseal MSCs that utilises the CD39/CD73 adenosine ectonucleotidase axis that may help maintain local immune homeostasis, while the same adenosine ectonucleotidase immunoregulatory pathway is likely dependent on Treg function in the intestine. This has broad implications for understanding the cellular bases of immune dysregulation in the gut-joint axis and could help guide tissue-specific therapy in SpA.
