Recent findings indicate that deficits in emotional regulation, stress and anxiety may influence lower sexual satisfaction, sexual dysfunctions and increased consumption of pornography. The objective of this research was to assess if traumatic stress and dissociative experiences from previous life manifested by typical symptoms will be related to increased levels of pornography consumption. In this observational study 1100 adult young men (mean age 27.55, age range 20-35, standard deviation = 5.21) were assessed for their pornography consumption behavior (sexual addiction screening test-revised, youth pornography addiction screening tool) and for their stress symptoms (trauma symptoms checklist-40), symptoms reflecting dissociative experiences (dissociative experiences scale), and symptoms of anxiety (self-rating anxiety scale). The results indicate that in these participants their pornography consumption experiences manifest significant positive Spearman correlations with stress symptoms (Spearman r = 0.47, P < .01), and mild correlations were also found for pornography consumption in its relationship to dissociative experiences and anxiety, and the results were also confirmed using Mann-Whitney test. The data indicate that the pornography consumption may be closely related to stress response, anxiety and defense mechanism related to dissociative experiences.

Sexual health is a fundamental component of quality of life that is frequently compromised in cancer patients. In melanoma care, these issues remain under addressed despite increasing survivorship. This scoping review aimed to map the current literature on sexual health concerns in melanoma patients. Following PRISMA-ScR guidelines, five databases were searched (2010-2025). Ten studies met the inclusion criteria, including cross-sectional, qualitative, and systematic reviews. Sexual dysfunction affected up to 52% of melanoma patients, with higher prevalence in men (68.9%) than women (41.3%). Contributing factors included visible scarring, hormonal imbalances (e.g., testosterone deficiency in 69% of men receiving immunotherapy), body image concerns, anxiety, and depression. Women were more affected by body image and recurrence anxiety, while men experienced higher rates of treatment-induced hypogonadism. In one cross-sectional study (n = 75), sexual dysfunction was reported in up to 52% of melanoma patients, with higher rates in men (68.9%) than women (41.3%); however, these estimates are based on limited data and should not be generalized.

The relationship between psoriatic arthritis (PsA) and Peyronie's disease (PD) remains poorly understood. PsA is a chronic immune-mediated inflammatory disease, whereas PD is a fibroproliferative disorder of the penile tunica albuginea that leads to penile curvature, pain, and sexual dysfunction. Both conditions involve chronic inflammation and dysregulated tissue remodeling. We report the case of a 42-year-old man with long-standing PsA who developed acute inflammatory PD shortly after discontinuing biologic therapy because of loss of insurance and medication nonadherence. The patient presented with new-onset penile pain and a palpable dorsal plaque in the setting of uncontrolled psoriatic disease. Penile ultrasound demonstrated fibrous plaque formation with linear calcifications consistent with early PD. Management included oral pentoxifylline, topical nitroglycerin, and resumption of systemic immunosuppression with secukinumab through a patient-assistance program, resulting in improvement in both penile symptoms and psoriatic disease activity. To contextualize this presentation, we performed a narrative review of the literature examining reported associations between PD and autoimmune or fibroinflammatory conditions. Prior studies have reported heterogeneous epidemiologic findings, with some suggesting an increased prevalence of psoriasis and PsA among men with PD, while others support a shared profibrotic biology mediated by transforming growth factor-β rather than by classic autoimmunity. This case highlights a potential temporal and pathophysiologic association between uncontrolled systemic inflammation and localized penile fibrosis and underscores the impact of social determinants of health on disease progression. Awareness of this possible association may prompt earlier urologic evaluation in patients with inflammatory arthritis and emphasizes the importance of maintaining continuity of biologic therapy in chronic immune-mediated disease.

Prostate cancer is the most common male malignancy, and although robotic-assisted radical prostatectomy (RARP) is widely used, urinary incontinence and erectile dysfunction remain significant issues. NeuroSAFE, an intraoperative frozen section technique aimed at optimizing nerve preservation without compromising cancer control, is increasingly used, but its overall impact on outcomes remains unclear.To systematically evaluate the effectiveness and safety of the NeuroSAFE technique during RARP, comparing functional recovery and oncologic outcomes with non-NeuroSAFE approaches.This systematic review and meta-analysis followed PRISMA 2020 guidelines and was registered in PROSPERO (CRD420251032774). A comprehensive search of Embase, MEDLINE, and ClinicalTrials.gov (March, 2026) identified studies reporting oncologic and/or functional outcomes in men undergoing RARP with NeuroSAFE. Two reviewer teams independently conducted study selection, data extraction, and risk-of-bias assessment using ROBINS-I tool. Random-effects meta-analyses estimated pooled odds ratios (ORs) with 95% confidence intervals (CIs). Heterogeneity was quantified with the I[2] statistic, and publication bias was evaluated using Egger's test.Thirteen studies (2 randomized controlled trials and 11 observational cohorts) comprising 22,183 patients met the inclusion criteria. NeuroSAFE was associated with significantly improved postoperative erectile function (OR 2.00; 95% CI 1.46-2.74) and urinary continence recovery (OR 1.36; 95% CI 1.05-1.76). Positive surgical margins were significantly reduced in the NeuroSAFE group (OR 0.73; 95% CI 0.59-0.89). No differences in biochemical recurrence were observed (OR 0.81; 95% CI 0.43-1.56). Heterogeneity was substantial for some outcomes, and most nonrandomized studies carried a serious risk of bias, which limits causal inference.This systematic review and meta-analysis, including randomized evidence, demonstrates that NeuroSAFE is a safe and effective intraoperative strategy that is associated with improved erectile function, enhanced continence recovery, and negative surgical margins without compromising oncologic outcomes. These findings support integrating NeuroSAFE into surgical decision-making for patients undergoing RARP, particularly when nerve preservation can be achieved without compromising oncologic outcomes.In this study, we compared a surgical approach that uses real-time analysis during prostate cancer surgery with the standard technique. This method helps surgeons decide how much tissue to preserve while the operation is still ongoing. We found that this approach is safe and may improve recovery of urinary continence and sexual function, while also reducing the risk of leaving cancer behind, without compromising cancer control.
