  • CONCLUSIONS: DOACs may provide comparable thromboembolic protection with a lower risk of major bleeding than VKAs after LAAO. However, most available evidence is derived from non-randomized studies and remains subject to residual confounding, selection bias, and moderate-to-low certainty of evidence. Therefore, the observed benefits of DOACs should be interpreted cautiously. Further large-scale prospective studies and randomized controlled trials are needed to confirm these findings and inform optimal post-LAAO anticoagulation strategies.

  • CONCLUSIONS: There were numerically more recurrent ICHs and fewer ischemic events in patients starting antiplatelet treatment after ICH compared with those avoiding antiplatelet treatment. The slow recruitment might be due to clinicians being certain of the best treatment strategy or patients being too frail to participate. STATICH will contribute to a planned collaborative individual patient-data meta-analysis to resolve this dilemma.

  • CONCLUSIONS: A substantial proportion of patients with ischemic stroke or TIA have a previously unrecognized risk of OSA, while excessive daytime sleepiness is relatively uncommon. STOP-Bang and ESS appear to capture different aspects of sleep-disordered breathing and sleep-related symptoms. STOP-Bang identified a larger proportion of patients at risk of OSA, whereas ESS provided complementary information on subjective daytime sleepiness. Routine screening for OSA symptoms may improve identification of modifiable risk factors relevant for stroke prevention.
