- CONCLUSIONS: This case highlights the prolonged latency between postpartum hemorrhage and presentation of Sheehan syndrome, with acute infection acting as a trigger for adrenal insufficiency. Clinicians should maintain high suspicion for Sheehan syndrome in women with a history of complicated delivery, lactation failure, amenorrhea, and chronic endocrine symptoms, even decades after the inciting event. Early recognition and prompt hormone replacement are lifesaving.

- CONCLUSIONS: Pipelle endometrial biopsy provides the highest rate of tissue adequacy in postmenopausal women; however, hysteroscopy-guided biopsy-particularly with grasper forceps-also yields acceptable adequacy and offers the advantage of targeted sampling. Cellular insufficiency appears to be the primary limitation to tissue adequacy in postmenopausal endometrium.

- CONCLUSIONS: Ovarian ectopic pregnancy is a rare but important diagnosis in early pregnancy and may be difficult to distinguish from other adnexal lesions. In this patient, transvaginal ultrasound and serum β-hCG findings supported the diagnosis of presumed ovarian ectopic pregnancy. Conservative management with methotrexate was chosen because the patient was hemodynamically stable and could undergo close follow-up. Serial β-hCG monitoring and follow-up ultrasound demonstrated progressive regression and complete resolution without surgery. This case highlights that carefully selected patients may be managed successfully with fertility-preserving medical treatment, even in a resource-constrained setting.
