- Advances in immune-inflammatory interactions within the testicular-penile microenvironment of varicocele patients: molecular mechanisms underlying erectile dysfunction and spermatogenic impairment. [Review]Front Endocrinol (Lausanne). 2026; 17:1813721.FE
- Varicocele is a prevalent cause of male infertility and has been associated with erectile dysfunction in clinical and preclinical studies, characterized by a complex pathophysiological process involving immune-inflammatory responses. This review focuses on the preclinically supported immune-inflammatory interactions between the testicular and penile microenvironments in varicocele patients, syste…

- Numerical simulation of gonadal vein hemodynamics in nutcracker syndrome-associated varicocele: effects of Valsalva maneuver and clinical implications. [Journal Article]Front Bioeng Biotechnol. 2026; 14:1828724.FB
- CONCLUSIONS: The Valsalva maneuver induces a high-pressure-negative-pressure-recompression vicious cycle across the LRV stenosis, with elevated upstream pressure increasing venous congestion risk and downstream negative pressure exacerbating stenotic collapse. The 12-fold WSS elevation far exceeds physiological thresholds, potentially precipitating cumulative endothelial injury. Recirculation zone formation compromises venous return and promotes thrombogenesis. Crucially, these hemodynamic perturbations are not confined to diagnostic maneuvers but are likely mirrored during daily activities that elevate intra-abdominal pressure (e.g., running, jumping), explaining the progressive symptom worsening and activity-related discomfort in NCS patients. These findings provide a hemodynamic rationale for dynamic clinical assessment.

- Predictive Factors for Successful Testicular Biopsy in Azoospermia [cite: 100]. [Journal Article]Tunis Med. 2026 Feb 01; 104(2):312-316.TM
- Testicular biopsy represents a crucial procedure in the management of non-obstructive azoospermia, though its success remains variable. [cite: 106] Identifying predictive factors for successful sperm retrieval is essential for patient selection. [cite: 107] Aim: To determine the clinical, hormonal and technical factors predictive of successful testicular biopsy in azoospermic patients [cite: 108]…

- Polyorchidism: A Narrative Review on Clinical Manifestations, Diagnostic Challenges, Malignancy Risks, and Therapeutic Considerations. [Review]Cureus. 2026 Apr; 18(4):e108046.C
- Polyorchidism, the presence of more than two testes, remains the rarest congenital anomaly of the male gonads, with fewer than 300 histologically or radiologically confirmed cases reported worldwide. Most supernumerary testes are discovered incidentally during imaging or surgery, yet their detection raises critical questions about malignancy risk, reproductive potential, and appropriate managemen…

- Health-Related Quality-of-Life Improvement After Microsurgical Varicocelectomy: A Prospective Observational Study Using WHOQOL-BREF. [Journal Article]Urol Int. 2026 May 30; :1-9. [Online ahead of print]UI
- CONCLUSIONS: Microsurgical subinguinal varicocelectomy provides early improvement in patient-reported quality of life independent of presenting complaint. These findings indicate that the benefits of varicocele repair extend beyond reproductive outcomes and support incorporating patient-reported outcomes into clinical counseling and treatment decision-making.

- Liquid embolic agents-a practical overview. [Review]
- Liquid embolic agents (LEAs) have expanded the therapeutic options for complex vascular and non-vascular conditions. Tailored clinical assessment with dedicated procedure planning is critical in the selection of the most appropriate embolic to ensure procedural success and minimise complications. The LEAs, which range from sclerosant agents to glues and copolymers all have specific physical chara…

- Immunological mechanisms and precision stratification in male infertility: from testicular immune privilege and danger signal amplification to seminal immune biomarkers and mechanism-tailored intervention. [Review]Front Immunol. 2026; 17:1806683.FI
- Male infertility presents significant heterogeneity, yet traditional assessments primarily rely on semen parameters and endocrine, imaging, and genetic screening. Although these methods can identify some definitive etiologies, a mechanistic explanatory gap persists in large populations with mild to moderate oligoasthenoteratozoospermia, elevated sperm DNA fragmentation, or recurrent adverse repro…

- Effect of adjuvant vitamin E supplementationon sperm parameters after varicocelectomy: a systematic review and meta-analysis. [Journal Article]Arch Ital Urol Androl. 2026 May 20. [Online ahead of print]AI
- CONCLUSIONS: Current evidence does not demonstrate a consistent improvement in key semen parameters with adjuvant vitamin E supplementation after varicocelectomy. Larger trials with standardized regimens and clinically relevant outcomes are needed.

- Evaluation of the efficacy of subinguinal antegrade sclerotherapy and subinguinal micro varicocelectomy versus a combined approach on the semen parameters in infertile patients: a randomized prospective comparative study. [Journal Article]
- CONCLUSIONS: Subinguinal MSV and SAS resulted in a significant post-operative improvement in the semen parameters. Nevertheless, combined approach should be recommended as it is associated with the lowest incidence of complications. Future studies with larger scales are needed to assert the current results.

- Spontaneous thrombosis of a left-sided varicocele in a diabetic adult: diagnostic approach and pathophysiological insights. [Journal Article]
- CONCLUSIONS: This case highlights the importance of differentiating primary from secondary varicoceles, excluding tumor thrombus, and recognizing the role of diabetes-induced endothelial dysfunction as a potential risk factor. Awareness of this rare entity can guide accurate diagnosis and management, preventing unnecessary intervention.

- Varicocele: new perspectives on an old problem. [Review]Fertil Steril. 2026 May 12. [Online ahead of print]FS
- Varicocele has a substantial adverse effect on sperm production and male fertility, so varicocelectomy has an important role in treating infertility. Beyond its effects on fertility, varicocele may also be associated with pain, affect testosterone production and have some relevance for men with the most severe forms of male infertility, non-obstructive azoospermia. The adolescent with varicocele …

- Nutcracker syndrome in 2026: a nephrologist-oriented diagnosis and management. [Review]Clin Kidney J. 2026 May; 19(5):sfag124.CK
- Nutcracker syndrome (NCS) refers to symptomatic compression of the left renal vein (LRV), most commonly between the aorta and superior mesenteric artery (anterior nutcracker) or, less frequently, between the aorta and vertebral column in the presence of a retro-aortic LRV (posterior nutcracker). This venous entrapment elevates renal venous pressure and promotes drainage through the gonadal and pe…

- Integrating bibliometrics and real-world data to map varicocele research and predict surgical outcomes. [Journal Article]Front Endocrinol (Lausanne). 2026; 17:1837765.FE
- CONCLUSIONS: Our findings confirm that microsurgical repair significantly improves semen parameters. We therefore advocate for a refined varicocele classification system that incorporates venous diameter, patient age, and reflux duration to enhance clinical decision-making and patient selection.
