  • CONCLUSIONS: Sapylin significantly reduces postoperative drainage volume following endoscopic radical thyroidectomy. However, its application is associated with a systemic inflammatory response and an increased risk of transient postoperative hoarseness. Although this nerve dysfunction is reversible, given the potential for neural irritation, its application should be restricted or approached with extreme caution, carefully weighing the benefits of drainage reduction against the risk of transient hoarseness.

  • CONCLUSIONS: The incidence of postoperative laryngeal paralysis was in line with international data, and none of the examined potential risk factors were able to prove their prognostic significance, so the personalized risk assessment still relies primarily on literature data in our practice.   • CONCLUSIONS: RLN preservation using IONM-guided shaving techniques can be a safe and effective alternative to nerve resection in selected patients with functional preoperative vocal cords. Partial nerve resection may also be feasible without compromising oncological safety. These findings highlight the importance of individualised surgical planning and suggest the need for standardised protocols in managing RLN invasion.
