The term transitions of care refers to the movement of patients between health care providers, settings, and home as their care needs and conditions change.[1] Transitions may occur in the outpatient or inpatient setting. Examples of care transitions encountered by OB/GYN hospitalists include handoff from the primary OB/GYN provider to the hospitalist on admission, handoff to another team when transfer to a higher level of care is necessary, or the transition of the patient from the hospital to home at discharge. Regardless of the scenario, transitions represent vulnerable points in patient care. OB/GYN hospitalists should understand the components of effective transitions and how to implement strategies to ensure reliable and smooth transitions of care for all patients.
